PRE-REGISTRATION

Upon Pre-registration, an estimate of your insurance related expenses will be provided for your upcoming visit. View a list of accepted insurances.

The Pre-registration process generally involves two components:  Pre-Registration by an admissions counselor and in certain cases Pre-Admission Testing. 

Prior to your surgery/procedure, an admissions counselor will call to obtain insurance information and demographics. If an Admission Counselor has not contacted you, please call the Admitting Department five days prior to your surgery at 512-654-0123. Please complete the below pre-registration forms and fax to 512-571-5312. This process will decrease registration time when you come to the hospital. Upon arrival in our Main Lobby, an Admissions Counselor will review the paperwork with you and answer any questions you may have.  Please ensure to bring a valid form of identification and your insurance card.

PRE-ADMISSION TESTING

Depending on your surgical procedure, you may be required to have some Pre-Admission testing such as blood work, x-rays, EKG, etc.  Your physician will indicate which tests are needed.  Pre-Admission testing is generally completed the week of the scheduled surgery.  However, it is not required for all procedures.  Please check with your Physician or ask our Admissions Counselor for If you call Monday through Friday between 7:30 a.m. and 4:00 p.m. and receive voice mail, leave your name and phone number, and you can expect a call within two hours. If you call after hours, we will call you the next morning.

Upon arrival for testing, please report to our Main Lobby Registration Department.  If you have already pre-registered, you will sign paperwork and be directed to the appropriate departments for testing.   If you have not pre-registered, please note the Admitting Counselor may be able to assist in completing your Pre-Registration prior to testing.

Who can request medical records or images?

A photo identification with name is required to obtain medical records from Baylor Scott & White Medical Center - Lakeway.  A signed Release of Information (ROI) form is required identifying the patient and/or the entity allowed to pick up the information on the patient's behalf.  ROI forms may be obtained and requested records picked up at the Information Desk located at the main entrance of the hospital.

Medical records or images may be picked up Monday-Friday between the hours of 8 a.m. and 4:30 p.m.  Please call 512-654-1226 for more information.




For your comfort and safety, there are several factors to keep in mind prior to your surgery or procedure. Please remember, these can affect the success of your procedure.  Unless otherwise instructed, the following instructions are required for anyone having surgery:

  • Follow eating and drinking limitations as instructed by your physician.
    -If you are having moderate sedation, you must strictly observe your time and requirements of nothing to eat or drink. It is hazardous to have anesthesia and surgery with anything in your stomach. Surgery may be canceled or delayed if you have taken any food or fluid.
    -If you are having only a local anesthetic, your surgeon will instruct you regarding eating and drinking limitations.
  • Please do not wear jewelry and do not bring valuable with you. Baylor Scott & White Medical Center - Lakeway is not responsible for lost valuables. Remove all body piercings and leave them at home.
  • If you bring your laptop computer or cell phone, the person accompanying you will take responsibility for those items.
  • Leave contact lenses and hair accessories at home.
  • Do not wear makeup or nail polish.
  • Pediatric patients will need a diaper bag and can have one small comfort item.
  • A friend or relative may wait during your surgery/procedure. 
  • If you develop any skin insult, any skin irritations such as ant bites, nicks, cuts, sun burns, etc., cold, fever, persistent cough or change in your condition during the week prior to your surgery, please notify your surgeon for treatment or referral.
  • Upon arrival, you will finalize admission procedures, sign consent forms and have your pre-operative evaluation. If the patient is a minor, a parent or legal guardian must be present to sign all forms.
  • State hospitals or state school patients must be accompanied by an attendant.

Upon arrival, please report to the Admissions Department in our Main Lobby.

If you arrive prior to 7:00 am for your scheduled procedure, please report to the Emergency Room entrance to obtain your registration paperwork. You will be escorted to the Surgical lobby.
